بیماران مختلف تجربه درد همزمان زانو و کمر را تجربه می کنند هر چند در مواردی ممکن است دو بیماری مجزا باشند ولی در مواردی نیز زانو درد ناشی از آسیب های کمری بوده و نیاز به درمان مجزائی ندارد.یک شناخت جزئی از آناتومی به ما می گوید که اعصاب مربوط به زانو از کمر منشاء می گیرند اعصاب جلوی زانو از ریشه های ۳و۴ و پشت زانو از ۵ و ریشه ۱ ساکرال می باشند لذا آسیب به این اعصاب در ناحیه کمر برای مثال آسیب های دیسک و تنگی نخاع می توانند منجر به زانو درد شود. بعلاوه در مواردی آسیب عصبی ممکن است بقدری شدید باشد که منجر به ضعف عضلات اطراف زانو گردد که این فرایند منجر به ناپایداری مفصل زانو و ایجاد آسیب و درد می گردد.
